Because TPTB like to ramp up tension, I imagine a demon or other similar baddie will tell Sam in order to mess with his mind (knowing he won't believe it and will probably get angry and sloppy fighting). Sam will wonder and wonder then Figure It Out. Shit will drip from the fan at that point.
