Dean's oblivious. He's Captain Oblivion. He might pick up on some vibe from Cass now and then but he's got so many other things on his mind now and Castiel's always been kind of weird so he just brushes it off.
He's not toying with Cass at all here, post-apoc Dean is a very no-nonsense kind of guy. He wouldn't even have the energy to fool around like that. He's just taking care of a very basic need in a very businesslike sort of way and Cass is just a guy to him now so he's lost whatever modesty he might ever have had around him. Of course the irony is that Cass being a guy now is exactly what's driving poor Cass crazy -- the angel Castiel could have sat through this little scenario without being remotely fazed.
